Use of PatientAccess for Health Care Services

By agreeing to these terms and conditions, I acknowledge that I am requesting access to portions of my health information and the ability to communicate with my VCU Dental Care health care team concerning my health information via the Internet using an electronic application called PatientAccess.

PatientAccess Username and Password

I understand that I will have a unique username and password to be used to access my health information via PatientAccess. The username and password in combination will identify me in PatientAccess. By using my username and password the system will know when I make inquiries and entries to my health record or another health record for which I've been granted access. I understand that it is extremely important I keep my username and password completely confidential. If at any time I feel that the confidentiality of my password has been compromised, I am responsible for changing it by clicking on the "Change Password" link under the "Personal Information" page. I understand that VCU Dental Care takes no responsibility for and disclaims any and all liability or consequential damages arising from a breach of health record confidentiality resulting from my sharing or losing my password. If VCU Dental Care discovers that I have inappropriately shared my password with another person, or that I have misused or abused my PatientAccess access privileges in any way, VCU Dental Care, without prior notice, may discontinue my participation in PatientAccess.

Security

Safeguarding your password and online identity is key to protecting your information. Suitable caution must be exercised in setting up Personal Verification Questions and Passwords. You must always keep your Personal Verification Questions and Answers strictly confidential.

Proxy Access

Proxy access allows you to access another patient's PatientAccess account. Proxy access can be used for parents who would like access to their children's records, or for adult patients wishing to grant access to other adults. As a proxy you can take advantage of PatientAccess features as if you were that person. In order to gain proxy access for your children, you must make this request in person upon checking-in for one of their outpatient visits. If the child is under the age of 15 the staff will be able to grant you proxy access. However, if the child is 15 years of age (or older) you and your child will need to complete the release of information form provided by the front desk staff. Once you have completed this form, the staff will be able to grant you proxy access to their records using PatientAccess. Similarly, adults wishing to grant proxy access to other adults will need to complete the release of information form before the proxy access can be granted.

Your Health Record

The health information that you are able to view using PatientAccess is taken directly from your electronic health record at VCU Dental Care.

I understand that PatientAccess may not be available to me at all times due to system failures, back-up procedures, maintenance, or other causes beyond the control of VCU Dental Care. Access is provided on an "as-is, as-available" basis and does not guarantee that I will be able to access PatientAccess at any particular time. During times when PatientAccess is unavailable, other communication methods (e.g., telephone) should be used.

I understand that VCU Dental Care takes no responsibility for and disclaims any and all liability arising from any inaccuracies or defects in software, communication lines, virtual private network, the internet or my internet service provider, access system, computer hardware or software, or any other service or device that I use to access PatientAccess.
